You may pre-register with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Complete and Send buttons at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we take every precaution to protect it.
Your First Visit
During your first appointment, your oral surgeon will discuss your diagnosis and treatment options, and offer plenty of opportunities for you to ask questions.
To ensure a smooth consultation, we attempt to collect the following prior to your appointment:
- Your referral slip
- X-rays (if applicable).
- Completed patient registration forms
- Insurance information
Additionally, let us know in advance of your appointment if you have a medical condition that may be of concern prior to surgery. Common examples include diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.
X-Rays
If your dentist or physician has previously taken X-rays, we will request they are forwarded to our office. If additional images are necessary, we will take these during your consultation.
